Frequently Asked Questions
To log-in to your Patient Portal, you will need to provide your Client Code, Account Number, and Password. This information is located on your billing statement.
The Client Code and Account Number are listed in the upper right-hand corner of the billing statement. The Client Code is a 3-letter code under "Client Code". To the right is the Account number, which is a 3-digit code under "Account Number."
If you do not have access to your billing statement and log-in information, you can call us toll-free at (855) 782-9118.
To make a payment on someone's behalf, you can do so online through the Patient Portal or by calling us toll-free at (855) 782-9118.
If paying online through the Patient Portal, you will need to provide the patient's Client Code, Account Number, and Password.
If calling us directly, you will need to provide the patient's full legal name and date of birth.
On-Call Specialist accepts American Express, Discover, Mastercard, and Visa.
